To accommodate the needs of our patients, particularly those who are working while receiving therapy, we offer early morning, evening and weekend hours, by appointment. We accept patient appointments on:

Checklist
- Please bring the doctor's prescription for physical therapy and your insurance card. We need to have these on file for billing purposes.
- Bring your date book for scheduling of future appointments.
- Plan on spending 45 to 90 minutes at our facility.
- Remember to wear comfortable clothing.
- Be prepared to pay your co-insurance/co-pay. We accept cash, check and credit card (Visa, MasterCard or Discover).
